Hi Dr. Andre. This is the end of my 12th day on a water fast. I'm feeling pretty good, but I've noticed that my eyes have become yellow tinted. Is this a detox symptom or something I should be concerned about?

If the sclera of the eyes become yellow dear Annie, it could be due to "jaundice" or high bilirubin in the blood. Bilirubin is eliminated by the liver into the upper part of the gut. If there's fiber in the gut, the bilirubin is bound to that and thus leaves the body. When fasting, the bilirubin tends to be re-absorbed which could lead to higher levels in the blood until such time as the gut contains fiber again to help eliminate it. Some of it will also leave via the kidneys so a urine dipstick in the urine might show up.

Even if this "jaundice" is the case, it may of course just be pure coincidence or a healing symptom but as a scientist I obviously like to know. If there's any opportunity to do a urine dipstick for bilirubin/urobilinogen and a blood liver function test that would be helpful.

Doctor, does it mean that people with elevated bilirubin level should add more fiber into their diet? Increased bilorubin is quite common in teenagers, i remember i used to chew on some very bitter seeds, i don't know the english either german name for, to lower bilirubin. Would more fiber help or is there anything else you would also recommend? I'd love to help my nephew.

An employee of mine looked jaundiced and her doctor recommended a liver panel. It showed nothing wrong. She told me, "Could it be from the carrots? I eat at least a dozen a day." She has a pseudo-jaundice from all the beta-carotene.

No dear Shant, you can catch up on bilirubin excretion quite easily when you eat again... just ensure that you have a high fiber diet. Bilirubin binds to fibers and is thus excreted instead of being re-absorbed.

ANd dear Haypeeoh, beta carotene tends to colour the skin but not the eyes. That's how we discriminate clinically between carotenaemia and jaundice. But indeed you can get yellow from carrots or, even more so, carrot juice. But carotene is not toxic so if she likes being yellow, so be it..
